Process of Professional Care Management

  • The process generally begins with a geriatric care manager visiting the place of residence to perform a functional assessment of the individual’s physical and safety needs, lifestyle choices, personality and financial needs

  • Primary concerns and recommendations are then shared with the designated family members, advisors and client (when appropriate) and addresses those in a priority order

  • A solution-oriented care plan is initiated when ongoing care management is requested

  • Whether the right solution is home care or an assisted living facility, professionally qualified, experienced caregivers and/or managers are selected
